الكساح Rickets
مــــاهـــــــــي أسـبــــــــاب لــــــــــيـن الـعـظـــــــــــــام ؟
أولاً : سوء التغذية .. نتيجة لعدم تناول كمية كافية من الطعام الذي يحتوي على فيتامين ( د ) أو الاعتماد على حليب الأم فقط بعد الشهر السادس من العمر .
ثانياً : الأطفال الخدج وناقصي الوزن عند الولادة .
ثالثاً : من لديهم أمراض مزمنة في المعاء أو الكبد أو الكلى .
رابعاً : قلة التعرض لأشعة السمش المباشرة .
في هذه الحالات يوصى بأخذ جرعة وقائية من فيتامين ( د ) .
Cause
The primary cause of rickets is a vitamin D deficiency. Vitamin D is required for proper calcium absorption from the gut. Sunlight, especially ultraviolet light, lets human skin cells convert Vitamin D from an inactive to active state. In the absence of vitamin D, dietary calcium is not properly absorbed, resulting in hypocalcaemia, leading to skeletal and dental deformities and neuromuscular symptoms, e.g. hyperexcitability. Foods that contain vitamin D include butter, eggs, fish liver oils, margarine, fortified milk and juice, and oily fishes such as tuna, herring, and salmon. A rare X-linked dominant form exists called Vitamin D resistant rickets.
Cases have been reported in Britain in recent years[5] of rickets in children of many social backgrounds caused by inability to make vitamin D because the sun's ultraviolet light was not reaching the skin because of persistent use of strong sunblock, or too much "covering up" in sunlight, or spending too much time indoors. Other cases have been reported among the children of some ethnic groups in which mothers avoid exposure to the sun for religious or cultural reasons, leading to a maternal shortage of vitamin D.[6][7] The British Medical Journal reported in 2010 that doctors in Newcastle on Tyne saw 20 cases of rickets per year

يظهر لين العظام تدريجياً وعلى مدة طويلة ، وقد لا يكون اكتشافه سهلاً للوالدين ، وهو يظهر في العادة بنهاية السنة الأولى وبداية السنة الثانية من العمر ، ومن أعراض المرض ما يلي :
1 - عدم انغلاق اليافوخ الأمامي في الرأس .
2 - تأخر ظهور الأسنان .
3 - تقوس الساقين .
4 - تشنجات بسبب نقص الكالسيوم .
Diagnosis
Rickets may be diagnosed with the help of:
* Blood tests:
o Serum calcium may show low levels of calcium, serum phosphorus may be low, and serum alkaline phosphatase may be high.
* Arterial blood gases may reveal metabolic acidosis
* X-rays of affected bones may show loss of calcium from bones or changes in the shape or structure of the bones.
* Bone biopsy is rarely performed but will confirm rickets

قد لا تكون العلامات السريرية واضحة في بداية الأمر ، ولكن مع حدوث لين العظام فيمكن ملاحظة التالي :
1 - عدم انغلاق اليافوخ الأمامي .
2 - الجبهة تكون عريضة .
3 - تقوس العظام .
4 - وجود عقد على الأضلاع .
5 - وجود اتساع في عظام الرسغ .
كـيـــــــــــف يـتـــــــــــــــم الـتـشـخـيــــــــــــص ؟
يتم التشخيص بالاعتماد على الأعراض والعلامات السريرية وتدعم بالفحوصات التالية :
1 - انخفاض مستوى الكالسيوم في الدم ، علما انه في بعض الأحيان يكون طبيعياً .
2 - انخفاض مستوى الفسفور في الدم .
3 - ارتفاع مستوى أنزيم الفسفات القلوي ( AIKaline Phosphatase ) .
4 - تغيرات في عظام الذراع بالأشعة السنية .
كـيــــــــــــــف تـتـــــــــــــــم الــوقـــايـــــــــــــــــــــة ؟
الوقاية خيرا من العلاج احرص على القيام بما يلي :
1 - تعرض الأم الحامل والمرضعة لأشعة الشمس لكي لا يكون لدينا نقص في فيتامين ( د ) مع الحرص على الغذاء الصحي .
2 - تعرض الطفل لآشعة الشمس المباشر في الصباح بين ( 10 صباحاً ــ 2ظهراً ) وبدون وجود الزجاج المانع لأشعة فوق البنفسجية .
3 - إضافة الطعام إلى حليب الأم عند الشهر السادس من عمر الطفل .
4 - استخدام الحليب المدعم بالفيتامين ( د ) والكالسيوم بعد الفطام من حليب الأم .
5 - الحرص على إعطاء الطفل ( 400 ) وحدة من فيتامين ( د ) خلال السنة الأولى من العمر أو مع الفيتامينات المتعددة للوقاية من لين العظام .
مـــــــــــــــــــــــــــاهــــــــــــــــو الـعـــــــــــــــــــــــــــلاج ؟
1 - جرعات عالية من فيتامين ( د ) حسب رأي الطبيب لمدة قد تتجاوز عن ( 6 أسابيع ) مع متابعة الفحوصات .
2 - علاج الأسباب وتعديل تغذيه الطفل .
3 - إن عدم الحرص على العلاج والمتابعة قد يكون لها تأثير على صحة الطفل في المستقبل

The treatment and prevention of rickets is known as antirachitic.

Ergocalciferol (D2)
Treatment involves increasing dietary intake of calcium, phosphates and vitamin D. Exposure to ultraviolet B light (sunshine when the sun is highest in the sky), cod liver oil, halibut-liver oil, and viosterol are all sources of vitamin D.
A sufficient amount of ultraviolet B light in sunlight each day and adequate supplies of calcium and phosphorus in the diet can prevent rickets. Darker-skinned babies need to be exposed longer to the ultraviolet rays. The replacement of vitamin D has been proven to correct rickets using these methods of ultraviolet light therapy and medicine.
Recommendations are for 400 international units (IU) of vitamin D a day for infants and children. Children who do not get adequate amounts of vitamin D are at increased risk of rickets. Vitamin D is essential for allowing the body to uptake calcium for use in proper bone calcification and maintenance.
[edit] Supplementation
Sufficient vitamin D levels can also be achieved through dietary supplementation and/or exposure to sunlight. Vitamin D3 (cholecalciferol) is the preferred form since it is more readily absorbed than vitamin D2. Most dermatologists recommend vitamin D supplementation as an alternative to unprotected ultraviolet exposure due to the increased risk of skin cancer associated with sun exposure. Endogenous production with full body exposure to sunlight is approximately 250 µg (10,000 IU) per day.
According to the American Academy of Pediatrics (AAP), all infants, including those who are exclusively are breast-fed, may need Vitamin D supplementation until they start drinking at least 17 US fluid ounces (500 ml) of vitamin D-fortified milk or formula a day.
